What do I need to do if I want a referral for an ADHD/Autism Assessment?
After you request a referral, we will need certain information before we can arrange a referral. This information is essential for all referral providers and helps to make the process as smooth and timely as possible.
The information demonstrates the concerns raised by you and others, and supports any future diagnosis.
We require the following:
1. Completed questionnaire
- AQ50 for autism, or ASRS for ADHD
- If you require both an ADHD and ASD assessment, please complete both forms.
2. Any supporting information, which may include:
- Old School reports
- Letters from family members about behaviours in childhood/adulthood
- Any previous private reports or diagnoses
- A personal statement about your symptoms and their impact on daily life
A referral cannot be made until all of this information is received.
Choosing a referral provider
- Many people choose a Right to Choose provider, as waiting times are often much shorter than with the NHS Adult Neurodevelopmental Service.
- The same information listed above is required whether you choose NHS or a Right to Choose provider.

Important considerations when choosing a provider
When looking at providers, please check carefully that:
- They assess the correct condition and age group
- They offer treatment/management as well as diagnosis
- If medication is recommended, it must be started and stabilised by the specialist before we can continue prescribing.
- We also need a formal shared care agreement with the provider.
- Annual reviews with the specialist are still required whilst medication prescribed
⚠️ If you choose a diagnosis-only provider, Lion Health cannot initiate treatment. You may have to restart the referral process elsewhere if you later decide to start medication.

We will then advise you on the next steps. You may not need a GP appointment for your referral to be processed.
Who Cannot Use Right to Choose?
You cannot use Right to Choose if you are:
- Already receiving mental health care for the same condition
- Referred to a service commissioned by a local authority (e.g., drug/alcohol services)
- Accessing urgent or crisis mental health care
- Accessing services through a primary care contract
- In high-secure psychiatric services or detained under the Mental Health Act (1983)
- Serving in the armed forces
