Help with a Health Problem – available 8am – 6.30pm Monday to Friday (use this to request any type of appointment for a new or ongoing medical issue including nurses appointments, vaccinations, smear tests, B12 injections, wound checks)

Or

An Admin Query – available 24hrs, 7 days a week (use this to request a repeat prescription, request a fit (sick) note, follow-up on a referral, request a doctor’s letter or use the simple ‘other admin request’ for anything else such as changing contact details
