What do I need to do if I want a referral for an ADHD/Autism Assessment?
After you request a referral, we will need certain information before we can book an appointment with one of our GPs. This information is essential for all referral providers and helps to make the process as smooth and timely as possible.
The information demonstrates the concerns raised by you and your child’s school, and supports any future diagnosis.
We require three things:
1. Completed questionnaire
AQ10 (child/adolescent) for autism in children aged 4-11 and 12-15, or SNAP-IV for ADHD
2. Letter from school
Outlining any concerns and observed behaviours/traits
3. Letter from parents/carers
Detailing your concerns and behaviours/traits you have observed
A referral cannot be made until all of this information is received.
Choosing a referral provider
- Many families choose a Right to Choose provider, as waiting times are often much shorter than with NHS CAMHS (currently around 3–4 years).
- The same information listed above is required whether you choose NHS CAMHS or a Right to Choose provider.

Important considerations when choosing a provider
When looking at providers, please check carefully that:
- They assess the correct condition and age group
- They offer treatment/management as well as diagnosis
- If medication is recommended, it must be started and stabilised by the specialist before we can continue prescribing.We also need a formal shared care agreement with the provider.
- Annual reviews with the specialist are still required whilst medication prescribed.
