This link will take you to another page which will give an option to complete one of two forms:
Help with a Health Problem – available 8am – 6.30pm Monday to Friday (use this to request any type of appointment for a new or ongoing medical issue including nurses appointments, vaccinations, smear tests, B12 injections, wound checks)
Or
An Admin Query – available 24hrs, 7 days a week (use this to request a repeat prescription, request a fit (sick) note, follow-up on a referral, request a doctor’s letter or use the simple ‘other admin request’ for anything else such as changing contact details
We are able to register new NHS patients if they live within our catchment area.
If you are a new patient looking to register with Lion Health, you can only do so if your address falls within the dark blue area shown.
