How do I obtain my blood tests results?

You can request test results using our online Test Results Triage. If there is any abnormality the surgery contacts the patient.

Can I change my appointment?

Yes you can, please call the appointment line to re-arrange, or use the Patient Access app/website to cancel and look for any further availability.

What time will my home visit be?

If asked for a visit that day and it is felt clinically appropriate then you will be visited that day by one of our ANP team, but an exact time can’t be given.

Can I order a prescription over the phone?

You can only order a prescription over the phone if you are a housebound patient.

How do I get a Blue Badge?

If you wish to request a blue badge you need to apply for this online using the below websites:

You can collect a paper copy of the form at all libraries and at Dudley council offices, and can call The Blue Badge Team on 0300 555 0055 for enquires.

How do I get an incontinence assessment?

You will need to organise an assessment with the community nursing team. You can call 01384 323170 to arrange this or we can leave a message in the DNs book for them to contact and arrange.

I’ve got a toothache, can I be seen as an emergency?

The GPs are not best to handle dental emergencies as they do not have the specialist training, you should go to a dentist. You can call 111 to find the nearest out of hours dentist.

Will a GP sign my passport?

GPs will not sign patient passports. These should be signed by a manager, teacher, or another person in a position of responsibility.

How can I get mobility aids?

Patients will need to speak with Occupational Therapy on 0300 555 0055.

How can I get a wheelchair?

You will need a referral from the GP to loan a wheelchair, free of charge, on a permanent basis.

Is the chemist open?

We are not affiliated with the pharmacy, but their opening time is 08:30, Monday to Friday and 09:00, Saturday.

Where is physio?

On the ground floor in the glass building at Lion Health next to the dentist.

Where do urine samples go?

There is a sample box in the reception lobby, all samples should be marked correctly, please ask reception for form and bag.

What time will my script be ready?

We ask for 2 working days and on the day it is ready it will be after 14:00.

Where do I put my prescription request?

There is a prescription box in the reception lobby, and another by the pedestrian gate nearest the building.

Where is the dentist?

On the ground floor in the glass building at Lion Health, next to physio/gym.
